Wound/Ostomy Management Program:
The VNA of LI's Comprehensive Wound/Ostomy Management Program:
- Enables case managers and discharge planners to efficiently plan home discharges
for clients with wound/ostomies.
- Provides physicians with the security that the client's wounds/ostomies will recieve
appropriate care at home.
- A Certified Wound/Ostomy Care Nurse is availible to make home visits.
- Allows the client with a wound/ostomy to safely live at home rather than remain in the
hospital or sub-acute facility, thereby improving the quality of life for the client
and their family.
Program Goals/Objectives:
- Early identification of those clients with wounds/ostomies.
- Provide the latest and most innovative wound/ostomy management treatements:
- With the assistance of the physician and certified wound/ostomy care nurse assess
and modify the treatment if wound/ostomy healing is not occuring.
- Identify and provide appropriate Home Medical Equipment to assist in
would healing.
- Prevention of infection.
- Prevent hospitalization and unnecessary visits to healthcare professionals.
- Collect information to track improvement
Program Specifics:
- All wounds/ostomies are assessed upon admission onto VAN of LI home health care program:
- All wounds/ostomies are staged, measured, photographed.
- Special instruments are used to assist the nurse in her assessment of the wound/ostomy,
such as a Doppler.
- The Certifed Wound/Ostomy Care Nurse will do an evaluation:
- Upon admission in the following circumstances:
- Diabetic-related wounds/ostomies
- Infected wound/ostomy
- Client with a wound/ostomy who is on an immunosuppresive treatment
- After 72 hours with twice a day wound/ostomy care with no change in treatement/status.
- On wounds/ostomies that:
- Haven't healed after 30 days
- Develop an infection
- Have recieved treatment for six months or more.
- For those clients needing infusion therapy at home, VNA of LI can coordinate these
services.
