LONG TERM HOME HEALTH CARE PROGRAM

The Long Term Home Health Care Program (LTHHCP) of the Visiting Nurse Association of Long Island provides an alternative to institutionalization for clients who otherwise would need placement in a skilled nursing or health related facility. This program, also known as the "Nursing Home Without Walls," is provided in conjunction with the New York State Department of Social Services.

 

A coordinated plan of care and services are provided at home to invalid, infirm or disabled persons with medical conditions who cannot function independently. This allows the client to remain in familiar surroundings with dignity and maximum independence. A professional team will provide a personalized treatment plan to meet each client's needs. This program is covered by Medicaid, Medicare in addition to Medicaid, or private pay.

 

LTHHCP's have a single focus: providing comprehensive services in the home for individuals who might normally be in nursing homes.

 

Some of the many services in the LTHHCP are:

  • Comprehensive case management
  • Skilled Nursing
  • Physical, Occupational, and Respiratory Therapies
  • Speech Pathology and Audiology
  • Home Health Aides
  • Medical equipment and supplies
  • Medical transportation
  • Medical social work services
  • Nutrition counseling

 

At least one of the following waivered services must be part of the client's careplan:

  • Respite Services
  • Assistive Technology
  • Community Transitional Services
  • Congregate and Home Delivered Meals
  • Environmental Modifications
  • Home and Community Support Services
  • Home Maintenance Services
  • Medical Social Workers
  • Moving Assistance
  • Nutritional Counseling/Educational Services
  • Respiratory Therapy
  • Social Day Care Transportation
  • Social Day Care

 

Client Eligibility

Recognized as a leader in home health care on Long Island, the VNA of LI offers a wide array of services to eligible individuals and families in Nassau, Suffolk or Queens Counties without regard to race, creed, color, age, sex, national origin, religion, sexual preference or disability.

 

  • Nassau, Suffolk and Queens Counties
  • Client needs can be met at home
  • Approval and supervision of attending physician
  • Has demonstrated a need for placement in a skilled nursing or health related facility
  • Availability of someone to give needed care between visits of home care staff
  • Needs coordination of services
  • Needs no more than 35-40 hours of personal care per week
  • Medicaid
  • Medicare in addition to Medicaid
  • Private Pay

Referrals for services may come from a variety of sources including physicians, hospital discharge planners, caregivers, family members, or individuals who are seeking care for themselves.
