 A coordinated plan of care and services are provided at home to invalid, infirm or disabled persons with medical conditions who cannot function independently. This allows the client to remain in familiar surroundings with dignity and maximum independence. A professional team will provide a personalized treatment plan to meet each client's needs. This program is covered by Medicaid, Medicare in addition to Medicaid, or private pay.  LTHHCP's have a single focus: providing comprehensive services in the home for individuals who might normally be in nursing homes.  Some of the many services in the LTHHCP are:
