The Visiting Nurse Association of Long Island's Certified Home Health Agency (CHHA) offers a wide range of comprehensive home health care services to clients and their families. Quality care is coordinated by our team of professionals to maximize a client's potential and to provide the necessary care and education in the home environment. Our professional care team includes Registered Nurses, Physical Therapists, Speech/Language Pathologists, Occupational Therapists, Nutritionists, Medical Social Workers and Certified Home Health Aides.

 

Our care team is supported by a variety of important staff and services to help clients be as independent as possible and to maintain a supportive home setting. At times the VNA of LI contracts with outside vendors to provide professional and support services for our clients.

 

Our CHHA is authorized to serve Medicare beneficiaries, Medicaid recipients and those in need of short-term, skilled nursing care. We provide:

  • Nursing services
  • Physical, Occupational and Speech therapies
  • Social work
  • Nutritional services
  • Home Health Aide services
  • Medical supplies
  • Equipment/appliances for home use

 

Client Eligibility

Recognized as a leader in home health care on Long Island, the VNA of LI offers a wide array of services to eligible individuals and families with medical or social needs in Nassau and Queens Counties, without regard to race, creed, color, age, sex, national origin, religion, sexual preference or disability.

Payment Options

Certified Home Health Agency services are covered in whole or part by:

  • Medicare
  • Medicaid
  • Private Insurance (including HMO’s)
  • Private Pay
  • Charity Care is available for those who qualify

Referrals

Referrals for services may come from a variety of sources including physicians, hospital discharge planners, caregivers, family members, or individuals who are seeking care for themselves.
