The Long Term Home Health Care Program (LTHHCP) of the Visiting Nurse Association of Long Island provides an alternative to institutionalization for clients who otherwise would need placement in a skilled nursing or health related facility. Meals On Wheels (MOW) is a not-for-profit program administered by the Visiting Nurse Association of Long Island and funded in part by the Nassau County Department of Senior Citizen Affairs, the New York State Office for the Aging and the U.S. Administration on Aging. MOW is dedicated to delivering nutritious meals to high-risk individuals over 60 with impaired mobility, and a limited number of individuals with disabilities under 60. The program is designed to provide nutritional education, related social services, information and referrals to community-based resources and services.Read more...
