      Professional Services


      VNA of LI offers a wide range of comprehensive home health services to clients and their families who reside in Nassau and Queens Counties. Quality care is coordinated by our team of professionals to maximize a client's potential and to provide the necessary care and education in the home environment. Our professional care team includes:

      Our professional team is supported by a variety of important staff and services to help clients become as independent as possible and to maintain a supportive home setting. These services include: At times VNA of LI contracts with outside vendors to provide professional and support services to our clients.

      Services are provided 24 hours per day, 7 days per week.

      Eligibility


      Recognized as a leader in home health care on Long Island, VNA of LI offers a wide rage of services to eligible individuals and families with medical or social needs in Queens and Nassau Counties, without regard to race, color, religion, national origin, sex, age, or disability. 

      Long Term Home Health Care Program

       Many elderly persons attest that their greatest fear is institutionalization. Medical conditions may make this inevitable for some people. But, our Long Term Home Health Care Program (LTHHCP) -- "Nursing Home Without Walls" -- can help people who live in Nassau, Suffolk, or Queens Counties to remain in familiar surroundings with dignity and independence for as long as possible.

      VNA/LI's LTHHCP services include all those provided by the Certified Home Health Agency, in addition to Audiology, Personal Care Aides, Homemakers/Housekeepers, Respiratory Therapy, a Personal Emergency Response System and certain services available by special arrangement.

      Ownership of this program was transferred to VNA/LI from the Nassau County Department of Health in 1996. The community's response to the program, which became operational during the second half of l996, has been gratifying. Through the close of 2001, 108 individuals had benefited from our LTHHCP. The current active census is 80. The program has a capacity to service 200 active clients.

       Home care has proved more cost effective than nursing home care. Therefore, programs such as the LTHHCP not only meet demonstrated need among our elderly residents, but may also significantly contribute to the statewide goal of reducing Medicaid expenditures.
       

      Meals on Wheels
      Meals on Wheels is available to elderly persons whose physical, emotional, mental or social condition restricts their ability to obtain or prepare meals for themselves, regardless of their income.

       Meals on Wheels is more than just a food service. Counseling and nutrition education is provided and participants are linked with other community agencies for additional services needed to maintain independence.

       Low sodium and diabetic diets and kosher are available. 

      Eligibility is established by the social work staff of the program, following guidelines of the funding agency. Service is given first to those who have the greatest need.

       Referrals
      Anyone may request the service: the person in need, relatives, friends, social service agencies, nurses, physicians, or hospital personnel.

       Meal Preparation
      Meals are prepared at local hospitals and a private kitchen facility. Delivery is up to 5 days per week, with a limited number of weekend meals available for the most vulnerable.

       Delivery of Meals
      Most meals are delivered by a network of volunteers shortly before or after 12 noon. The delivery consists of one hot and one cold meal, providing two thirds of the Recommended Dietary Allowance.

       Meals cannot be left if the recipient is not at home. Meals cannot be provided unless there is adequate refrigeration for the cold meal.

       Length of Time of Service
      Service can be provided short or long term, for partial or full week and intermittent respite. Need is established at an initial home assessment visit. Reassessments are made every six months thereafter.
