At Home Health Monitoring Program

At Home Health Monitoring Program:
Honeywell HomMed Health Monitoring Program

The VNA of LI's At Home Health Monitoring System:

  • Enables case managers and discharge planners to safely discharge home clients who need close clinical monitoring.
  • Enhances all specialty programs by daily monitoring of vital signs and collecting data on case specific questions.
  • Provides physicians with "up to the minute" clinical reports including frequently faxed copies of the following vital signs: weight, blood pressure and oxygen saturation.
  • Allows the client to maintain a stable health status, thereby inproving quality of life.

Program Goals/Objectives:

  • Early identification of those clients needing additional monitoring.
  • Empower the client to manage his/her illness at home.
  • Improve compliance with medications.
  • Prevent hospitalization and unnecessary visits to healthcare professionals.
  • Collect information to track improvement.

Program Specifics:

  • The Intake Nurse identifies clients appropriate for the program.
  • With the physician's approval we are able to provide the monitoring system.
    • The physician establishes the monitoring parameters.
    • The monitoring system is installed into the client's home and the client/caregiver is taught how to work the equipment.
  • Every day at 11AM the client/caregiver is to do the vital sign check, answer the programmed questions and transmit the data.
  • The Central Station Nurse reviews all transmissions and handles all Red Alerts.
  • Red Alerts are:
    • Vital signs falling outside of the set parameters.
    • Unfavorable responses to the programmed questions.
  • If no data is received by 11:30AM the Central Station Nurse calls the client.
  • If the client is feeling poorly, he/she can do the vital sign check and transmit the data any hour of the day or night.
  • Verbal and written communication is maintained with the physician at regular intervals.
  • The Visiting Registered Nurse determines if the client needs the MedPartner for medication compliance.
    • The MedPartner is programmed to remind the client when to take medications.
    • The MedPartner can either be provided with the monitoring system or can be installed separately.
