As a result of an earlier stroke and cardiac problems, Mr. M had limited mobility which resulted in a repeated skin breakdown condition. He was on many medications including a blood thinner which was difficult for his wife to administer. In addition, due to his limited mobility, Mr. M needed assistance with his activities of daily living. With assistance from the Long Term Home Health Care Program, a nurse pre-poured Mr. M's medications, evaluated Mr. M's cardiac status, assessed his skin integrity and communicated with his doctor regarding his activities of daily living. During this time, a nurse's aide was also sent to assist Mr. M with personal care and other daily living activities, a nutritionist offered nutritional counseling, and a social worker provided supportive counseling for Mr. M and his wife. Today Mr. M's cardiac status is stable, he is following his medication regiment, and he has had no further episodes of skin breakdown. We are pleased to report that Mr. M and his wife now have frequent "lunch dates" at a local diner after a morning nurse's aide has gotten him ready for the afternoon.
