Wound/Ostomy Management Services:
The VNA of LI's Comprehensive Wound/Ostomy Management Services:
- Enables case managers and discharge planners to efficiently plan home discharges for clients with wound/ostomies.
- Provides physicians with the security that the client's wounds/ostomies will receive appropriate care at home.
- A Certified Wound/Ostomy Care Nurse is available to make home visits.
- Allows the client with a wound to safely live at home rather than remain in the hospital or sub-acute facility, thereby improving the quality of life for the client and their family.
Goals/Objectives:
- Early identification of those clients with a wound.
- Provide the latest and most innovative wound/ostomy management treatments:
- With the oversight of the physician, our certified wound/ostomy care nurse assesses and may modify the treatment if wound healing is not occurring or if the ostomy site/ostomy care requires a change in treatment/management
- Identify appropriate medical supplies to assist in wound healing/ostomy management.
- Assist clients/caregivers to learn appropriate ostomy management skills and techniques.
- Prevent hospitalization and unnecessary visits to healthcare professionals.
- Collect information to track improvement
Specifics:
- All wounds are assessed upon admission onto VNA of LI's home health care program. All wounds are measured and staged upon identification.
- The Certified Wound/Ostomy Care Nurse will do an evaluation:
- Upon admission in the following circumstances:
- Diabetic-related wounds
- Infected wound/ostomy
- Client with a wound/ostomy who is on an immunosuppressive treatment
- After 72 hours with twice a day wound care with no change in treatment/status.
- On wounds that:
- Haven't healed after 30 days
- Develop an infection
- Have received treatment for six months or more.
- On ostomies where:
- Client is having difficulty with ostomy6 management.
