Mr. M, an 84 year old gentleman who lives with his wife, received Certified Home Health Agency services for a skin breakdown condition called Sacral Decubitus. Although Mr. M's skin problem healed with time, his pre-existing medical condition led to an ongoing need for medical attention.

As a result of an earlier stroke and cardiac problems, Mr. M had limited mobility which resulted in a repeated skin breakdown condition. He was on many medications including a blood thinner which was difficult for his wife to administer. In addition, due to his limited mobility, Mr. M needed assistance with his activities of daily living.

 

With assistance from our Long Term Home Health Care Program (LTHHCP), a Nurse pre-poured Mr. M's medications, evaluated Mr. M's cardiac status, assessed his skin integrity and communicated with his doctor regarding his activities of daily living. During this time, a Nurse's Aide was also sent to assist Mr. M with personal care and other daily living activities. A Nutritionist offered nutritional counseling, and a Social Worker provided supportive counseling for Mr. M and his wife.

 

Today Mr. M's cardiac status is stable, he is following his medication regiment, and he has had no further episodes of skin breakdown. We are pleased to report that Mr. M and his wife now have frequent "lunch dates" at a local diner after a morning Nurse's Aide has gotten him ready for the afternoon.

L.C. was was born with hydrocephalus (water on the brain) and a congenital skin condition that causes lesions to grow on her skin and into her internal organs. She was also born with a seizure disorder. L.C.'s mother is a single mom who used to only speak Spanish. L.C. and her mother live together in a studio apartment. Our Long Term Home Health Care Program staff have been able to provide services to L.C. and her mother that have greatly enhanced their overall quality of life.

A Nurse visits L.C. weekly to check on her medical status and the functioning of the shunt that was put in to relieve the pressure from hydrocephalus on L.C.'s brain. The Nurse checks to find out if the seizure medication L.C. has been taking is controlling her seizures or if the physician must be contacted to adjust the medication.

A Home Health Aide visits for four hours, three times weekly, to care for this little girl. The Home Health Aide reads stories to her and plays games with her. During this time, L.C.'s mother attends English as a second language classes. These classes have allowed L.C's mom to better communicate with health professionals and be an advocate for her daughter.

Through the Long Term Home Health Care Program L.C. also receives Physical and Occupational Therapy. This is very important for her congenital conditions which have resulted in developmental delays. Both Physical and Occupational Therapy are helping L.C. to overcome these delays.

The program also helps to arrange for transportation to L.C.'s medical appointments. Prior to being on this home care program, L.C.'s mother had to rely on public transportation to get L.C. to her appointments. As a result, today L.C. and her family are enjoying greater independence as well as sustaining healthier and happier lives.
