Certified Home Health Agency

The VNA of LI’s Certified Home Health Agency delivers a wide range of services to the

residents of Nassau and Queens counties. Our mission is to provide the best quality home health and community based services while doing everything possible to ensure that clients remain as independent as possible. Our Registered Nurses coordinate a multidisciplinary health care team

of physical and occupational therapists, speech/language pathologists, social workers, nutritionists and home health aides who work together to provide that special kind of support that facilitates the healing process.

Long Term Home Health Care Program

The VNA of LI’s Long Term Home Health Care Program, also known as our Lombardi Program

or Nursing Home Without Walls, helps clients who live in Nassau, Suffolk and Queens counties to remain at home with dignity and independence within the home setting. Providing the same quality services similar to those provided by our Certified Home Health Agency, our Long Term Home Health Care Program also offers the additional capacity of Respiratory Therapy, Audiology, Personal Care Aides, homemakers/housekeepers, a Personal Emergency Response System,

and other services by special arrangement.

 

Meals On Wheels

Meals On Wheels provides services within portions of Nassau County to those individuals whose physical, emotional, mental or social conditions restrict their ability to obtain or prepare a meals for themselves. Counseling and nutrition education services are also provided to encourage these individuals to take an active roles in their health and well-being. Service areas include the following towns:
