REGISTERED NURSES
Position: Registered Nurses

Location/Terms:

  • Per-Diem RN'S needed for both Queens and Nassau counties.
  • Homecare RN"S needed for Maternal Child Health Program for Prenatal/Post Partum Visits. Fluency in Spanish preferred.

Job Description:
Responsible for the provision and coordination of high quality home care services to clients on the Home Health Care Program consistent with the Agency's mission/vision/values, Policies and Standards of Nursing Practice.

  • Responsible for the coordination, assessment and evaluation of the physical, psychological, social/environmental and economic needs of all clients assigned initially and on an ongoing basis.
  • As the Coordinator of Care, develops, coordinates, implements and evaluates in conjunction with the client/caregiver and physician a multi-disciplinary care plan in order to meet the identified need of the client.
  • Renders safe, effective nursing care based on sound scientific principles and the nursing process.

REHABILITATION SERVICES
 Positions:
  • Physical Therapists

Location/Terms:
  • Fee-for-Service positions available in Nassau and Queens Counties.

Job Description:
Provide quality services to the clients of the Home Health Agency in the home environment.

  • Assesses and evaluates the rehabilitation needs of the client through observation, interviewing and physical examination initially and on an ongoing basis.
  • Develops and implements in conjunction with the client/caregiver, physician, RN Coordinator of Care and Rehab Manager, a comprehensive care plan in order to meet the identified needs of the client.
  • Responsible for the provision and coordination of high quality home care services to clients on the Home Health Care Program consistent with the Agency's mission/vision/values, Policies and Standards of Nursing Practice.

MEDICAL SOCIAL WORKERS

Location/Terms:

  • Fee-for-Service positions available in Nassau and Queens Counties.

Job Description:
Provide social work services to the clients of the home health agency in the client's home environment in accordance with the agency's mission/vision/values.

  • Assesses and evaluates assigned client's psychosocial, cultural, environmental and economic needs initially and on an ongoing basis.
  • Develops and implements a comprehensive treatment plan to meet the identified needs of the client in conjunction with the client/care givers/physician and R.N./Coordinator of Care.
